Ignite classes are delivered by Ignite Dance Coaching, owned and operated by NicoleDavies (referred to in this document as “Ignite Dance Coaching”).

 

I, the parent or legal guardian, declare that the participant named below is under 18 years of age and that I consent to their participation in Ignite Dance Coaching classes instructed by Nicole Davies.

 

I acknowledge and understand that the classes are designed for dancers and may involve physical activity including but not limited to strength training, conditioning exercises, stretching, balance work, jumping, and bodyweight exercises.

 

I understand that participation in these activities may be physically demanding and involves inherent risks, including but not limited to muscle soreness, strains, sprains, falls, or other physical injury.

 

I acknowledge that participation in the classes is voluntary and that the participant is not obliged to take part in any activity they do not wish to do. I understand that the participant may stop participating at any time and will follow any instructions given by Nicole Davies regarding safety or cessation of activity.

 

I declare that I have informed Nicole Davies of any medical condition, injury, illness, or physical limitation that may affect the participant’s ability to safely participate in the classes. I acknowledge that it is my responsibility to ensure the participant is physically capable of taking part in these activities.

 

I understand that Nicole Davies does not provide medical advice and is not a medical professional. I acknowledge that Nicole Davies shall not undertake any obligation to diagnose, advise on, or treat any injury or medical condition.

 

I consent to the participant receiving basic first aid or emergency assistance if required during in-person classes. I acknowledge that Nicole Davies is not medically trained and that emergency services may be contacted if necessary.

 

Online Participation

 

I acknowledge that Ignite Dance Coaching classes may be delivered online. By allowing the participant to take part in online classes, I accept responsibility for ensuring that the participant has a safe and suitable space to exercise, free from hazards, and that appropriate supervision is provided where necessary.

 

I understand that Nicole Davies cannot control the participant’s exercise environment during online classes and accepts no responsibility for injuries that may occur as a result of unsafe surroundings, equipment, or lack of supervision.

 

Assumption of Risk and Release of Liability

 

I acknowledge that these classes constitute a recreational service and that participation involves inherent risks.

 

To the fullest extent permitted by law, I agree that Ignite Dance Coaching shall not be liable for any loss, damage, injury, or expense suffered by the participant as a result of participation in Ignite Dance Coaching classes, whether delivered in person or online.

 

I release and hold harmless Ignite Dance Coaching and its owner, Nicole Davies, from any and all liability, claims, demands, or causes of action arising out of or related to the participant’s involvement in the classes.

 

Personal Property

 

I acknowledge that Ignite Dance Coaching and its owner, Nicole Davies, shall not be liable or responsible for any personal belongings that are lost, damaged, or stolen during in-person classes.

 

Acknowledgement

 

I confirm that I have read and fully understand this waiver and release of liability. I voluntarily agree to the terms and conditions stated above on behalf of the participant.

 

I acknowledge that this waiver may be pleaded in response as a bar to any legal proceedings brought by or on behalf of the participant.
